Customization of SIRS Forms

Schools

Schools have the ability to customize their SIRS form as long as Questions 9 and 10 (the overall course quality and teaching effectiveness questions) are retained. Please contact our office at otear@rutgers.edu or by phone at (848)-932-7466 to discuss and make arrangements.

Departments

Departments can add questions to the standard SIRS form. These questions will then be automatically loaded into the prearranged course surveys. In DIG you will see a field called Survey Form (circled in the picture below) which denotes what additional questions will be added to the standard SIRS questions.

If your department would like to add questions please contact our office at otear@rutgers.edu or by phone (848)-932-7466 to discuss and make arrangements. Questions will need to be received about one month after the start of a semester to be added to course surveys for that semester.

Below is a list of the departmental survey forms in the Blue system. Click on the name of a form to see the questions. In some of the questions you will see “[C$FN] [C$LN]” – this is a piping code that will place the instructor’s name into the question. If there are two or more instructors the question will repeat with each instructor’s name inserted.

Instructors also have the option of adding their own questions before the survey is sent to the students. They may choose from an existing optional question bank, or enter questions of their own wording.

The standard SIRS questions are included on department surveys, with the exception of the “Guest Lecturer (short form)” which replaces the standard SIRS. All other departmental forms listed below are questions in addition to the standard SIRS.
